Application and Registration News
Below are our Spring 2009 applications in RTF format. Click thelinks below to download, then open the file in your word processor,print, complete in black pen, and send it in.
No registration is complete and should be considered received on time until we have processed all three required pieces
- Part 1) Fully completed registration form with name, address, contact phone, valid email address, and uniform size.
- Part 2) Parent Pledge signed, read and understood by the family of each player.
- Part 3) Registration fee must be received via a personal or cashiers' check. We do not accept credit cards.
All registrations must be mailed to the league by each family orgiven to the player's Coach for them to submit to Subdivision Soccer.
No forms or fees will be accepted at or by Dean's Dugout cardshopfor Subdivision Soccer. Any registration left there is not consideredreceived by the league and will be subject to a late fee. Thank you foryour cooperation and continued support of Subdivision Soccer.
